The literature review presents current information on the classification of matrix metal- loproteinases and highlights their multifaceted role in the regulation of various physiologi- cal processes. At the same time, matrix metalloproteinases are considered to be sufficiently informative biomarkers for many diseases, including cardiovascular, oncological, neurologi- cal, rheumatological, and various inflammatory processes. Emphasis is placed on the fact that matrix metalloproteinases are important modulators of inflammation and metabolism due to their numerous multidirectional effects. An important evolution in understanding the biology of matrix metalloproteinases has been the realization that, despite their name, they are not simply destroyers of matrix proteins. It is highly likely that they also ensure the functioning of mechanisms that act as an additional and quite significant factor in the regulation of intercel- lular and intracellular communication. Among other things, it is noted that these endopepti- dases are quite influential factors in mediating numerous immune defense reactions. A general description of the biological significance of tissue inhibitors of metalloproteinases is provided. Information from the latest literature sources on the status of several matrix metalloproteinases and their inhibitors in patients with infectious diseases of the respiratory system and certain inflammatory processes is analyzed.
