Abstract

During the full-scale Russian invasion of Ukraine since 2022, there has been an increase in cases of antibiotic-resistant infections, especially in areas of active hostilities. This is due to the military receiving mine and explosive and gunshot wounds during shelling. Such wounds are usually deep and often contaminated with soil, debris, and biological contaminants, mak- ing treatment with standard antimicrobials difficult. Almost always, the wounded are infected with persistent multidrug-resistant pathogens, such as: Methicillin-resistant staphylococcus aureus, Acinetobacter baumannii, Pseudomonas aeruginosa, Klebsiellapneumoniae, Candida albicans), especially when evacuation occurs hours or even days after the injury, which significantly complicates the treatment of infections, and leads to loss of limbs and sometimes fatalities. The treatment of military personnel with various types of wounds is also complicated by the presence of polymicrobial and complex combined infections with contamination by foreign fragments, which require an integrated approach (antiseptic and surgical treatment). The main drawback of therapy during the treatment of various types of wounds in the military is the insufficient effectiveness of antimicrobial and antifungal drugs, since traditional drugs do not cope due to the resistance of pathogenic strains and, in screening, often turn out to be poorly adapted to the conditions of field hospitals. There is also a problem, namely the lack of protocols for combined antimicrobial and antifungal treatment that would be adapted to combat and field conditions. The need to develop the composition and dosage form of an innovative combined medicinal product exists and remains relevant.
